Complete Intake Form

Meet with Your SLP
Your licensed speech-language pathologist conducts the screening and provides a clear summary with next-step recommendations.
Key Milestones for Ages 5–6
By kindergarten, most children should demonstrate these foundational speech and language skills.
Clear Speech
Speaks clearly enough to be understood by most listeners, including unfamiliar adults, at least 90% of the time.
Complex Sentences
Uses sentences of 5–8 words, including conjunctions like 'because,' 'but,' and 'so' to connect ideas.
Narrative Skills
Can retell a simple story with a beginning, middle, and end — a foundational skill for reading comprehension.
Phonological Awareness
Identifies rhymes, counts syllables, and begins to isolate individual sounds — critical for early literacy development.
